Application forms

Blue card application forms and
Exemption card application forms (for registered teachers and police officers)

Our new form which can be used to complete a BC, BCB, E or EB form is now available. The form has built-in smarts to make sure you give us all the information we need on the first go. You will still need to have your identification verified so make sure you follow all of the instructions, including printing the form at the end.

If you prefer to fill the form manually (the old way) that’s ok too! You can still access the individual forms below. 

Blue card application forms

A Blue card application (BC) form (PDF, 226KB) should be lodged by paid employees, volunteers and trainee students proposing to start or continue in child-related employment.

A Blue card business application (BCB) form (PDF, 228KB) should be lodged by self-employed persons or executive officers proposing to start or continue to carry on a child-related business.

Exemption card application forms (for registered teachers and police officers)

Registered teachers and police officers providing child-related services that fall outside of their professional duties do not require a blue card. They should instead apply for an exemption card.

An Exemption card application (E) form (PDF, 223KB) should be lodged by paid employees, volunteers and students.

An Exemption card business application (EB) form (PDF, 218KB) should be lodged by business operators, including directors of a non-state school governing body.

Where an organisation is unable to sight the applicant’s identification documents, please obtain the Confirmation of identity form and submit together with the application form.

Child protection (including foster and kinship carer) forms

Blue card application forms for:

  • foster carers
  • kinship carers
  • adult members of a foster carer’s or kinship carer’s household
  • paid employees of a Licensed Care Service, and
  • nominees and directors of Licensed Care Services

can be obtained from the Department of Child Safety, Youth and Women - Child Safety Services at https://www.csyw.qld.gov.au or by telephoning 3239 3521.

A Blue card application (BC) form (PDF, 226KB) should be lodged by volunteers at a Licensed Care Services and by paid employees, volunteers or trainee students carrying out work inside a Licensed Residential Facility.

Where an organisation is unable to sight the applicant’s identification documents, please obtain the Confirmation of identity form and submit together with the application form.

Disability services application forms

A Blue card application (BC) form (PDF, 226KB) should be lodged by people providing disability services to children only.

A Blue card business application (BCB) form (PDF, 228KB) should be lodged by persons carrying on a regulated business which provides disability services to children.

A Blue card application and request for yellow card exemption (Y) form (PDF, 214KB) should be lodged for applicants proposing to start or continue work with both children and adults, at a place where disability services are provided by:

  • the Department of Communities, Disability Services and Seniors (the Department) funded non-government service provider or,
  • a National Disability Insurance Scheme (NDIS) non-government service provider.


After an application for a blue card has been finalised, relevant information regarding the application will be forwarded to the Department of Communities, Disability Services and Seniors to process the request for a yellow card exemption.

Where an organisation is unable to sight the applicant’s identification documents, please obtain the Confirmation of identity form and submit together with the application form.

Family day care and Stand alone care application forms

Family day care

 

Family day carers/assistants

Family day care educators or paid assistants in a family day care service should complete the Family day care educator/assistant blue card application (FDC) form (PDF, 214KB) and pay the prescribed fee.

Family day care adult residents/occupants

Adults (i.e. people over the age of 18 years) residing in a home where family day care is being provided should complete Adult resident/occupant blue card application (AO) form (PDF, 207KB).

There is no application fee for adult residents/occupants as they are considered to be volunteers.

Stand alone care

 

Stand alone carers

Stand-alone carers who are self-employed should complete the Blue card business application (BCB) form (PDF, 228KB) and pay the prescribed fee.

Self-employed persons providing stand-alone care are also required to develop and implement a child and youth risk management strategy. More information and a suite of tools to assist people to develop and strengthen their risk management strategies can be found on the Child and youth risk management strategies page.

Stand alone care adult residents/occupants

Adult occupants of a home in which a stand-alone care service is being provided can apply for a blue card/ exemption card using the Adult resident/occupant blue card application (AO) form (PDF, 207KB).

There is no application fee for adult residents/occupants as they are considered to be volunteers. Volunteers are required to hold a blue card prior to the stand-alone carer commencing their stand-alone care service.

Supporting forms

Confirmation of identity form (where an organisation/employer cannot sight ID)

A Confirmation of identity form (PDF, 119KB) (formerly known as the Prescribed person identification verification form) should be completed by a prescribed person where a blue/exemption card applicant cannot have their identification sighted by their organisation because either:

  • the applicants usual residence is more than 50km from the business address of the organisation, or
  • the applicant has a disability affecting their mobility.


This form must accompany the blue/exemption card application form.

Request to consider alternative identification (where an applicant cannot satisfy the ID requirements)

The Request to consider alternative identification form (PDF, 161KB) is to be completed by blue/exemption card applicants who cannot satisfy the identification requirements when making an application for a blue/exemption card.

This form must accompany the blue/exemption card application form.

Statutory Declarations (to accompany application forms)

Director of a governing body of a non-state school

A director of a non-state school governing body is deemed to be carrying on a regulated business and can waive the application fee by submitting a Board Member - Waive Fee Statutory Declaration (PDF, 52KB) together with the Blue card business application (BCB) form (PDF, 228KB).

Change of name

Where a document evidencing a name change cannot be provided, a Name change evidence - statutory declaration (PDF, 54KB) should accompany the Change of Name form (PDF, 172KB).
